Hemofiltration Solutions
HF-solutions are needed for the continuous renal replacement therapies (CRRT) with the multiFiltrate. They are used mostly as substitution solution in hemofiltration therapy, but can also be used as dialysis fluid in case hemodialysis is applied.
Besides the contribution to the removal of uraemic toxins and the correction of the patient's fluid status, HF-solutions also correct the acid-base-balance of the patient. As renal failure leads to an exhaustion of the buffer systems and, consequently, to the development of metabolic acidosis, the net administration of buffer base is required.
Bicarbonate: direct correction of acidosis
With the use of bicarbonate-buffered hemofiltration solutions a direct increase of the buffer capacity in the blood occurs.
The multiBic solution from Fresenius Medical Care was tested in a large multi-centre, prospective randomised study with 117 patients by Barenbrock et al. (Kidney Int 2000, 58:1751) resulting in:
- increase of the buffer capacity
- significantly improved correction of the metabolic acidosis
- significantly less hypotensive episodes
- less cardiovascular complications
The use of bicarbonate as the physiological buffer substances in hemofiltration solutions requires state-of-the-art technology to guarantee safety and maximum handling convenience.
